Jowls and a sagging jawline: skin, fat, ligament or bone, and what lifts what

Dr Ong Jin Khang · MMC 45245 · LCP holder · The Retreat Clinic, Setia Alam

The jawline loses its line with age because four layers change at once. The skin thins and loses recoil, the fat compartments of the lower face shift and deflate, the ligaments that hold that fat loosen so a jowl forms, and the jaw bone itself loses height and changes angle. Faces carry these in different proportions, and the proportions decide what helps. Energy devices act on the skin and the tissue under it, filler adds support, botulinum toxin releases a muscle that pulls down, and surgery repositions tissue when laxity is marked. In the trials the effects of the non-surgical options are modest.

A jawline is read in one glance, and built in four layers. A treatment has to match the layer.

Why does the jawline lose its line with age?

Because four layers change at the same time, and the jawline is where the changes add up. The skin thins and loses recoil. The fat of the face, which sits in separate compartments, shifts and deflates. The ligaments and spaces that hold that fat in place loosen, and the fullness slides down to form a jowl. The jaw bone itself loses height and changes angle.

Faces carry these changes in different proportions, so two people can point at the same jowl with different anatomy behind it. The proportions decide what helps, and the rest of this article takes the layers one at a time.

What happens to the skin?

A review of facial ageing from the bone outwards describes weakening and thinning of the skin as the outermost change, over fat, muscle and bone that are changing underneath. Thin skin with less recoil drapes where it used to hold. When this layer dominates, the jawline can look soft even where the structures beneath have moved little.

What do the fat compartments do?

The fat under facial skin is not one layer. In dissections of 30 half-faces, the subcutaneous fat was partitioned into discrete compartments, and jowl fat was the lowest of them. A second study of 18 half-faces found fibrous septa forming the walls between the compartments, and concluded that the face ages in three dimensions, with compartments changing in position and volume relative to one another.

In practice some compartments descend and others thin. A descended pad gathers along the jaw as a jowl, and a deflated one leaves hollowness above it. Both can read as sagging, and they call for different answers, one that lifts or releases and one that supports. Both studies were done on cadavers, which show the structure and cannot show the change over time in one living face.

Which ligaments hold the jowl?

Ligaments and spaces tether soft tissue to the bone and set where it is free to move. A dissection study of 16 fresh cadavers, together with observations from several hundred facelift operations, described a membrane-lined space over the lower masseter, the premasseter space, reinforced by retaining ligaments. With age the collagen in its roof falls significantly, and the authors report that the space expands as its front and lower boundaries weaken, and that this expansion forms the jowl. The buccal fat beside it descends too, and adds to the fold between the mouth and the jaw.

That is anatomy from cadavers and an operating theatre. Surgeons treat the space as a natural plane for lower facelifts, which is one reason a jowl that has settled sits beyond what a device reaches.

Does the jaw bone change?

It does, and the change can be measured. CT scans of 120 adults in three age bands showed that the height of the ramus, the height of the body of the jaw and its length all fell with age in both sexes, while the angle of the jaw widened. The width across the angles did not change significantly. A review adds that the area of the jaw just in front of the jowl is one of the sites with a strong tendency to resorb.

Less bone under the same covering leaves the skin with more surface than it needs, and the border blurs. The scans came from Caucasian adults, people without teeth were excluded, and different people were compared across ages, so the figures describe a pattern and not the bone of any one reader.

How can you tell which layer dominates?

A mirror suggests, and an examination decides. The clinic's assessment reads the face layer by layer. The skin is pinched to judge recoil, the descent of the deeper support is checked at rest and in movement, and the volume that props everything up is checked separately, because a hollow cheek can drop a fold that looks exactly like laxity. The sagging skin page sets out the three faces that call themselves saggy: loose, descended and deflated.

Rough pointers follow from that. Skin that folds easily and settles slowly points to the skin. A jowl on an otherwise full face points to descent. A lean face with hollows and a blurred border points to deflation and bone. A jaw that was never sharply defined is a skeletal matter. Most faces carry more than one.

What do energy devices lift?

Energy devices heat tissue at a chosen depth, and the body answers by remodelling collagen over the following weeks. Micro-focused ultrasound (Ultherapy) delivers energy at points down to the deeper support layer, and radiofrequency (XERF) heats a broader volume nearer the surface. How the device classes compare is covered in its own article.

The trials show a real and modest effect. A meta-analysis of 42 studies of micro-focused ultrasound found that investigators judged 89% of patients to show some improvement, and its authors warn that counting "neutral" answers as positive may overestimate the benefit. It was funded by a manufacturer. A randomised trial of 51 people, given a single treatment to the lower face and neck, found a gain of more than one point in jawline laxity scores, with lifting on three-dimensional photographs. An earlier blinded cohort raised the brow by 1.7 mm on average, and its authors noted that lower-face tightening could not be measured because that area has no fixed landmarks.

For radiofrequency, a systematic review of 23 articles found measurable improvement in facial laxity with an acceptable complication profile. Most complications were minor and short-lived, and major ones were reported more often with monopolar than with bipolar devices. On the clinic's pages, micro-focused ultrasound suits early to moderate laxity, the lift builds over two to three months and typically lasts up to a year, and radiofrequency has no proven long-term durability figure yet. Neither moves a settled jowl back up the face.

What does filler do for jowls and bone loss?

Filler adds support and does not tighten skin. Where the jaw or the fat has deflated it replaces some of what is missing. In a trial of 15 women with moderate jowls, filler placed at the angle and the back of the jaw improved jowl scores at four weeks, using an average of 3.36 mL, and chewing was tender for two to three days. That is a small trial with a short follow-up.

Chin and jawline filler sets out what the gel can change, how long it lasts and the vascular risk. Thread lifts reposition tissue mechanically with a suture, and why the clinic does not offer them lays out the evidence on how long they last.

Where does botulinum toxin fit?

Only where a muscle is pulling. The platysma, the thin sheet of muscle in the neck, shows as vertical bands and a less defined jawline when it contracts. In a placebo-controlled phase 3 trial, onabotulinumtoxinA (sold as Botox), improved platysma prominence by at least one grade in 76.7% of the affected group against 21.2% on placebo at day 14, and the responder rates declined gradually towards day 120. The manufacturer funded the trial.

Toxin acts on a muscle, so it does not tighten skin, move fat or replace bone. Whether a given product is registered for use in the neck in Malaysia is worth asking before agreeing. At this clinic, platysma bands are treated with botulinum toxin when the assessment shows the muscle is the cause.

How are the options combined at this clinic?

At this clinic, botulinum toxin and filler are used for jowls when the assessment shows they are indicated, often combined with an energy device: micro-focused ultrasound (Ultherapy) or radiofrequency (XERF) for the skin and the support beneath it, or OndaPro, whose face handpiece treats fat under the chin and along the jawline. The plan follows the layers found at the assessment. The trials above tested each option on its own, so the size of a combined effect has not been measured.

When is surgery the honest answer?

When laxity is marked, with excess skin or an established jowl. The clinic's position is that no energy device gives a result worth its cost at that point, and that no machine repositions tissue the way a surgeon does. A surgeon assesses surgery, and a skin clinic refers. Skin that is loose after large weight loss has its own limit, set out in loose skin after weight loss.

The honest complication

Most of what is known about these layers comes from cadavers, scans and small trials. The treatment trials follow people for weeks to a few months, grade the result on scales the investigators chose, and several were paid for by the manufacturers. The lower face is hard to measure at all. The four routes have not been compared head to head on the same jawline in the studies cited here. A plan is a reasoned estimate made after examining the face, and it can change as the face responds.

Common questions

What causes jowls?

Jowls form when several layers change together. Cadaver studies describe the fat compartments of the lower face shifting, the premasseter space beside the jaw expanding as its boundaries weaken, buccal fat descending, and the skin thinning over a jaw bone that loses height and changes angle. The mix differs from one face to another.

Can jowls be lifted without surgery?

To a modest degree, if the problem is early laxity. In trials, micro-focused ultrasound improved jawline laxity scores by more than one point after a single treatment, and filler at the angle of the jaw improved jowl scores at four weeks in a small study. Marked laxity with excess skin or an established jowl is a surgical question.

How do I know if my sagging jawline is skin, fat, ligament or bone?

A mirror can only suggest it. An examination reads the layers one at a time: how well the skin recoils when pinched, how far the deeper support has descended at rest and in movement, and how much volume is missing. Most faces carry more than one cause.

Does botulinum toxin lift the jawline?

Only where a muscle is pulling. In a placebo-controlled trial of people with prominent platysma bands, a single treatment improved the bands and jawline definition at day 14 by at least one grade in 76.7% of treated people against 21.2% on placebo. It does not tighten skin, move fat or replace bone.
