Barbie Botox, lip flip and baby Botox: the viral injection trends checked

Dr Ong Jin Khang · MMC 45245 · LCP holder · The Retreat Clinic, Setia Alam

Barbie Botox, the lip flip, baby Botox, the Botox brow lift, the Nefertiti lift and masseter slimming are all botulinum toxin (Botox) used to weaken a chosen muscle, and each has a specific risk the videos rarely show. The trial data are small: 20 women for the trapezius, 17 for the lip flip. This is education, not a recommendation.

A viral look is a muscle switched down on purpose.

What are the viral injection trends, and what do they have in common?

Six names keep appearing on TikTok and Xiaohongshu: Barbie Botox (or trap Botox), the lip flip, baby Botox, the Botox brow lift, the Nefertiti lift and masseter slimming. They share one mechanism. Botulinum toxin (Botox) blocks the signal that makes a muscle contract, so the muscle works less for a few months. Each trend picks a different muscle and relies on the change in shape when it relaxes.

The videos show the shape. They rarely show that every muscle has another job. That is why each section below names the muscle, the evidence found for it, the specific risk and who should not have it. This page is education about what has been published, not advice for any one face.

Is Barbie Botox the same as the "Barbie drug"?

No, and the search results mix them up. Barbie Botox is a muscle-relaxing injection. The "Barbie drug" is the nickname for melanotan II, an unlicensed injection sold online to tan the skin, which has nothing to do with botulinum toxin.

The two share a nickname and little else. The related peptide PT-141 and the unregistered products around it are covered in PT-141 and skin darkening. The rest of this page is about the muscle-relaxing injections.

What is trap Botox, and what does the evidence show?

Trap Botox is botulinum toxin injected into the trapezius, the muscle that slopes from the neck to the shoulder, so that the neck looks longer and the shoulder line slimmer. It is the muscle behind the "Barbie Botox" label.

The only randomised trial found is small. In a 2026 randomised, double-blind trial from Seoul, 20 women received 40 or 80 units spread over 10 points along the trapezius. At 12 weeks the muscle area was smaller in both groups (p = 0.011 and p = 0.005), the median satisfaction score was 4, and the abstract reports no complications or difference between the two doses. That is 12 weeks in 20 women, which says nothing about repeated courses over years.

What are the risks of trapezius injections?

The trapezius is not only a contour. It helps to hold the shoulder blade steady and to lift the shoulder, so weakening it can change how the shoulder moves. A 2007 case report describes a 49-year-old woman who had repeated injections to both upper trapezii for muscle pain. After the third set she developed shoulder impingement linked by the authors to the weakness, and it settled within 3 months after the injections stopped.

That is one case, and the injections were for pain, so it does not give a rate. It does show a risk that is a function of the muscle, not of the brand. Neck and shoulder weakness, a heavier-feeling arm when lifting and shoulder pain are the things to ask about. Anyone who lifts, swims, climbs or works with arms overhead has a stronger reason to ask first.

What is a lip flip, and why can straws and whistling get harder?

A lip flip is a few units of botulinum toxin placed in the orbicularis oris, the ring of muscle that closes the lips, so the upper lip rolls out slightly and shows a little more red. A 2025 systematic review found 48 articles, of which 7 gave injection sites and doses, and the usual dose was 4 to 6 units.

A 2026 study of 17 women using 4 units measured a modest rise in upper lip height at 15 days (p = 0.011) and no change in lip volume, so the lip is not made fuller, it is tipped outwards. The side effects were mild and temporary, including numbness and a functional limitation, and they cleared within 30 days. The review lists difficulty whistling and drinking through straws, resolving within weeks.

The reason follows from the muscle. Weakening the lip-closing ring can make straws, whistling, some speech sounds and playing a wind instrument harder, and if the weakness is more than intended, control of liquid at the lip. Drooling was not measured in either source, so this is the mechanism and not a rate. A smile that shows a lot of gum is a different question, covered in the gummy smile article.

What is baby Botox?

Baby Botox is an informal name for smaller doses of botulinum toxin, aimed at softening movement and keeping some expression instead of stopping it. It is not a product and it has no fixed dose.

No trial read for this page defines "baby" by a number of units, so the term cannot tell you how much is going in. What matters is the dose per muscle, the muscle itself and the interval before review. The question of when to start, and whether starting early prevents lines, is set out in what age to start anti-wrinkle injections or skinboosters.

What is a Botox brow lift, and does it work?

A Botox brow lift relaxes the muscles that pull the brow down, so the muscle that lifts the brow, the frontalis, is left comparatively stronger and the outer brow sits a little higher. The change is small and depends on where the brow starts.

It also carries the risk that goes with the forehead: treating the frontalis too much can make the brow feel heavy or the upper lid droop. The article on hooded eyes without surgery sets out what a brow lift can and cannot do, and Botox side effects covers the eyelid and brow effects.

What is the Nefertiti lift?

The Nefertiti lift places botulinum toxin along the jawline and into the platysma, the thin muscle of the neck that pulls the lower face down, with the aim of a slightly sharper jaw and neck line. A 2022 Italian consensus of ten specialists included it, which tells you it has been in use, not that it has been proved by trials.

A 2026 commentary adds the reason for caution: the platysma comes in different patterns, and when the pattern is not suited, weakening it may give minimal lift or even more visible sagging, so the author argues for a moving examination before any injection and not a fixed grid. It is a commentary, not a trial. Neck bands are a more clearly defined target, and loose neck skin and neck bands explains when toxin, XERF or Ultherapy is the question.

Does masseter Botox slim the face, and what is the catch?

It can, where the muscle is the reason the jaw looks wide. In an 80-adult trial from Korea, masseter thickness at clenching fell by 2.33 to 3.79 mm at 12 weeks across four doses, all beating placebo, with no significant adverse events, and the authors concluded at least 48 units was enough. The trial adults had a mean age of 42.7 and mostly women, so it does not tell you about your jaw.

The catch is that a wide lower face can be bone, fat, loose skin or a muscle, and toxin only changes the last. Chewing may feel weaker for a while. The clinic's anti-wrinkle injections include the jaw, and masseter injections for jaw slimming and teeth grinding sets out the mechanism and evidence. Dr Ong performs injectables at this clinic.

Who should not have these injections?

The usual exclusions apply to every trend above: pregnancy and breastfeeding, a neuromuscular condition such as myasthenia gravis, an infection in the area, an allergy to the product, and some medicines, including certain antibiotics. Who should not have anti-wrinkle injections lists them in full.

Each trend adds its own reasons to wait or say no. A person whose work or sport depends on shoulder strength has a reason to ask about the trapezius. A person who plays a wind instrument, sings or speaks for a living has a reason to ask about the lip flip. A person whose brow already sits low has a reason to ask about the brow lift. A doctor may reasonably say no to a trend and offer another route.

The honest complication

The trials found are small, short and mostly of women, and two of the six sources are a case report and a commentary. Nothing here shows that any trend is safe over years, and none of the sources read compares a trend with doing nothing over a lifetime. A trend going viral is evidence that people want it, not that it suits you. What to ask any injector is plain: which muscle, how many units, what the muscle does besides the look, how long the effect lasts, and what happens if it is not right.

Common questions

What is Barbie Botox?

Barbie Botox, also called trap Botox, is botulinum toxin injected into the trapezius muscle, the sloping muscle between the neck and the shoulder, to make the neck look longer and the shoulder line slimmer. It is not the same as the Barbie drug, which is an unlicensed tanning injection.

Is the lip flip safe?

The studies read describe mild, temporary effects, such as difficulty with straws and whistling and some numbness, settling within weeks, but the evidence is small. The muscle that is weakened is the one that closes the lips, so whether it suits you depends on how you use your mouth.

What is baby Botox?

It is an informal name for smaller doses of botulinum toxin, used to soften movement rather than stop it. There is no fixed dose behind the name, so ask how many units are going where.

Does trapezius Botox cause shoulder weakness?

It can, because the muscle is a shoulder stabiliser. A case report describes shoulder impingement after repeated trapezius injections that settled within 3 months. The one randomised trial of 20 women reported no complications, but it was small.

Is the Botox brow lift or the Nefertiti lift worth it?

Both are small, subtle changes that depend on how your brow, jaw and neck muscles behave, and neither suits everyone. A doctor should examine you moving before deciding, and may advise against it.

Can masseter Botox slim the face?

A 12-week trial of 80 adults found the masseter thinner at every dose tested, by about 2.3 to 3.8 mm when clenching. It slims the jaw only where the muscle, not the bone or fat, is the reason it looks wide.
