Quick answer
Ultherapy Prime is a newer Ultherapy console. The FDA record shows a redesigned console, larger screen and updated software, with identical ultrasound energy. Ask for the device model, search the MDA register, and check the provider and transducers are genuine.
- Same technology: micro-focused ultrasound with real-time imaging, interchangeable transducers, depths of 1.5, 3.0 and 4.5 mm.
- Changed: console design, an 18.5 inch screen in place of 15 inch, Windows 10 in place of Windows XP, and later software with wireless connectivity and a planning mode.
- Comfort: two small studies report lower pain with Prime; neither is a controlled trial, and one had Merz funding.
- Genuine check: the model on the MDA register, a Certificate of Authenticity, authorised transducers, and a named doctor.
What is Ultherapy Prime, and how is it different from the older Ultherapy system?
Ultherapy is the treatment and the Ulthera System is the device, as the guide to Ulthera, Ultherapy and HIFU explains. Prime is the name of the newer console. The US FDA cleared it in February 2024 as the Ulthera System (UC-1 Control Unit PRIME), and the clearance summary calls the change modifications to the console, measured against the 2018 system.
Later records add software, not a new technique. A May 2025 clearance covers software for wireless connectivity and a treatment planning mode called SEE.PLAN.TREAT. Other clearances extended the indications to the abdomen and arms in February 2025 and to the knees in May 2026. Merz has also said it plans to end full service and support of the older platform on 30 September 2027, as reported by Practical Dermatology, which is one reason clinics are changing consoles.
Ultherapy Prime and the earlier console: what does the FDA summary compare?
The table lists only what the FDA summary for the 2024 clearance states.
| Feature | Prime console (K233996) | Earlier console (K180623) |
|---|---|---|
| Ultrasound signal energy | Identical | Identical |
| Treatment guidelines | Identical | Identical |
| Main components | Console with touchscreen, handpiece, interchangeable transducers | The same |
| Imaging | Imaging mode; visualisation up to 8 mm with the DeepSEE transducer | The same |
| Indications in 2024 | Lift the brow, lift lax tissue under the chin and on the neck, improve lines and wrinkles of the decolletage | The same |
| Screen | 18.5 inch, 1920 by 1080, widescreen | 15 inch, 1024 by 768 |
| Operating system | Windows 10 | Windows XP |
| Software version | 2.1.2030 | 1700 |
| Weight | Up to 12.2 kg | 10 kg |
What did not change?
The treatment itself. Both consoles deliver low-intensity, highly focused ultrasound below the skin, producing small points of thermal coagulation that lead to tightening, and the FDA summary lists the energy and the treatment guidelines as identical. The transducers stay interchangeable, and they set the depth: a consensus paper describes three depths, 4.5, 3.0 and 1.5 mm, and the imaging lets the doctor confirm skin contact and avoid bone.
The planning questions also stay with the person. The same consensus found 800 to 1,200 lines optimal for the full face and neck, customised to face size, laxity and the ultrasound image. That is why what an Ultherapy fee is made of turns on lines, area, transducer, a registered device and the doctor, and not on the console name.
Does Ultherapy Prime hurt less or work better?
Two small studies report lower pain, and neither can prove it. In a Bangkok questionnaire of 231 people who had been treated with both consoles, pain scored 5.59 out of 10 with the older one and 2.75 with Prime, redness 3.23 and 1.61, swelling 2.97 and 1.31. The author notes a single centre, unblinded treatment, no crossover design and a need for studies elsewhere.
In a Singapore case series of 30 Asian adults with one session, the mean pain score was 4.50 with a topical anaesthetic cream, and at day 90 86.7% had improved neck volume sagging and 70.0% improved jawline sagging by at least one point on the manufacturer's scale. The series had no comparison group, and its authors were Merz consultants. The FDA summary found the ultrasound energy identical, so a difference in results is not what the regulator recorded. For comfort in general see does Ultherapy hurt.
Is Ultherapy Prime the same as HIFU?
In the words of Dr Ong, HIFU is a copy of Ultherapy: both use focused ultrasound at set depths, but the FDA summary stresses that Ultherapy, with either console, can also image the tissue while treating, to a depth of up to 8 mm, to confirm skin contact and a safe depth. Many lower-priced HIFU machines are different devices from different makers, and the sources read do not rank them against Ultherapy. The guide to Ulthera, Ultherapy and HIFU sets out the differences, and the device name is the first thing worth asking about.
How do I check an Ultherapy device and its transducers are genuine in Malaysia?
Five questions cover it, and a clinic that is proud of its device can answer all five.
- Which device and model, and what is its registration? Search the Malaysian Medical Device Register at mdar.mda.gov.my. A marketing name such as Prime is not a registration. The 2026 medical device order explains why the register matters for HIFU.
- Can I see the Certificate of Authenticity? Merz advises patients to seek certified providers who can present one, and points to the provider map on its Ultherapy Asia site, as reported by The Star.
- Where were the device and transducers bought? In the May 2026 raid, a counterfeit Ultherapy Prime unit and 25 counterfeit transducers were seized from a clinic that had not bought from Ulthera Inc or its authorised distributor in Malaysia. The name on an advertisement proves nothing, as before you book a low-priced HIFU explains.
- Who holds the handpiece? Since 1 June 2026 the person operating a HIFU device needs a designated medical device permit, and a doctor's registration can be checked using the guide to checking an aesthetic doctor.
- What does the quote cover? Lines, area and transducer should be named, so two quotes can be compared on the same terms.
Who performs Ultherapy at The Retreat Clinic in Setia Alam?
Dr Ong performs Ultherapy himself, after an assessment of whether it suits the face and the sagging. This article describes the manufacturer's newer console and does not say which Ultherapy system any clinic runs, so the five questions above are worth asking of every clinic, this one included. The Ultherapy page sets out the treatment.
