Quick answer
Cheek filler adds volume to a midface that has flattened or hollowed, and it supports the tissue above it. It does not tighten skin and it does not lift jowls on its own.
- What it does: replaces volume lost over the cheekbone and in front of it.
- What it cannot do: tighten loose skin or lift jowls; those depend on skin, ligaments, fat and bone.
- Evidence: in a controlled trial in China, 76% of treated adults improved at 6 months against 8% of untreated controls.
- Duration: 15 to 24 months in one trial, depending on the part of the cheek.
- Risks: tenderness, swelling, firmness and lumps; rarely a blocked blood vessel. Too much filler gives a heavy, rounded look.
What does cheek filler do?
Cheek filler replaces volume lost from the midface and gives support to the tissue that rests on it. With age the fat under the cheek shifts and thins and the bone behind it changes, so the cheek flattens.
The gel is hyaluronic acid. Where it goes matters: the trials divide the cheek into the outer cheekbone area, the front of the cheek and the area below the bone, and the correction lasted for different lengths of time in each. How it feels is covered in does filler hurt lips and cheeks.
Who suits cheek filler?
Cheek filler suits a midface that has lost volume, which the trials call midface volume deficit, rated moderate to severe in the large US trial. That trial enrolled people aged 35 to 65, and the trials in China enrolled adults with midface deficiency, so the evidence comes from adults who had a measurable loss of volume, not from people wanting a different face shape.
It does not suit a problem that is not volume. Loose skin, a jowl formed by loosened ligaments, or a sunken look from bone change each needs a different answer, and a doctor works out which layer is the cause by examining the face at rest and in expression.
What can cheek filler lift, and what can it not lift?
Cheek filler can lift the look of a flat or hollow midface, and it cannot lift jowls or tighten loose skin by itself. A small trial of 15 women with jowls compared filler at the angle of the jaw alone with filler at the jaw and the outer cheek. The jaw-only group improved by every blinded reviewer after 4 weeks, with 3.36 mL on average. The group that also had cheek filler, 8.38 mL on average, did not reach significance by the blinded dermatologist, though the women rated it as improved.
That is a very small study over four weeks, so it shows that more filler in the cheek is not automatically what a jowl needs. The full picture, including energy devices and surgery, is in jowls and a sagging jawline.
Does cheek filler suit a hollow face after weight loss?
It can, for the volume part. Cheeks, temples and the area under the eyes are where many people first notice a gaunt look after fast weight loss, and filler is one of the options for the volume that has gone. It does not address loose skin. Ozempic face and Mounjaro face sets out what changes and what helps.
What do trials report for cheek filler?
In a randomized, evaluator-blinded trial in China, 111 adults had a hyaluronic acid filler in the cheeks and 37 had no treatment. At 6 months 76% of the treated group had improved by at least one point on a midface volume scale, against 8% of controls, and 51% were still responders 12 months after the last injection. Imaging showed a volume gain close to the 3.4 mL injected, stable through 12 months. Two of the authors work for the maker.
A second Chinese trial, with 119 treated and 27 untreated adults, found malar volume rose by 1.83 mL at 6 months with a median of 2 mL, against 0.11 mL in controls, and that 98.2% (investigator) and 93.8% (participant) rated the cheeks improved. Allergan employees are among its authors. Both measured volume and ratings, not how natural the result looked.
How long does cheek filler last?
In a controlled trial of one product, correction lasted between 15 and 24 months depending on the part of the cheek, with similar total volumes: 24 months for the front of the cheek, 19 for the outer cheekbone area and 15 for the area below it. In the same 235-person trial, 92.8% rated their cheek volume improved at 6 months and 79.0% at 2 years. The trial was funded by the maker.
Trial durations are not a promise for any one face, and the gel can remain in the tissue after it stops being visible. How long does filler last covers the product, area and tissue movement.
What are the risks of cheek filler?
The common effects of cheek filler are tenderness, swelling, firmness and lumps or bumps, mostly mild to moderate and lasting up to about two weeks in the 235-person trial. Bruising is also common. Late lumps and late swelling can happen and are covered in filler gone wrong.
The rare serious risk is a blocked blood vessel, with skin that turns white, grey or blue, unusual pain or any change in vision. That is an emergency. Dermal filler risks lists the warning signs. Hyaluronic acid filler can be dissolved with hyaluronidase, which is the usual reversal.
How much cheek filler is too much?
Too much looks heavy: rounded, pushed forward or distorted. The published term is facial overfilled syndrome, described as the result of excessive filler and suboptimal technique, with "chipmunk" cheeks and "pillow" faces among its forms. A review by authors including a Kuala Lumpur clinician reports it mainly in people of East Asian descent and says fillers cannot entirely reverse ageing.
Its advice is to assess the face first, watch facial movement during injection, use less filler, and consider bone and tissue changes and energy devices before adding more. The wider question of faces drifting towards the same template is in why does everyone look the same now.
Where can I have cheek filler near Setia Alam?
The Retreat Clinic in Setia Alam lists cheeks among the areas of facial contour its dermal fillers cover, and Dr Ong performs fillers himself, after examining the face. Ask what is being replaced, how much, and what happens if a vessel is affected.
