Quick answer
Primary underarm sweating is treated in steps. Antiperspirants come first, and botulinum toxin injections are the option when they do not control severe sweating. The effect wears off, and the injections can be repeated.
- First line: antiperspirants, according to two reviews of treatment.
- Injections: sweat fell by at least half in 94% against 36% on placebo at 4 weeks (320 people, BMJ 2001).
- Duration: median 197 to 205 days in a 52-week trial of 322 people; a Chinese trial followed 16 weeks.
- Dose on the US label: 50 units per underarm, in 10 to 15 skin injections after the area is mapped.
- The fee follows the product, the units, the assessment, who injects and the follow-up.
What is the first treatment for underarm sweating?
The first treatment is a topical antiperspirant. One review of treatment options states that antiperspirants are regarded as first-line therapy for primary focal hyperhidrosis, and another says mild sweating may be controlled with them and lifestyle changes. Hyperhidrosis affects nearly 4.8% of the US population, and 93% of cases are idiopathic primary focal, which means no other disease explains it.
The Botox label matches this order: it is indicated for severe primary underarm sweating inadequately managed with topical agents. A doctor who sees you can check that the sweating is primary and discuss antiperspirant strength and use first. Botox for excessive sweating sets out the full ladder and the warning signs.
Does Botox work for underarm sweating?
Botulinum toxin type A reduces primary underarm sweating in trials. In a trial of 320 adults, 50 units per underarm gave a sweat fall of at least half in 94% against 36% on placebo at 4 weeks, and 82% against 21% at 16 weeks. In the 322-patient study on the US label, a fall of more than half occurred in 81% on 50 units against 41% on placebo.
A Chinese phase 3 trial of 344 adults, reported in 2026, found 83.72% against 55.81% at week 4 after one 50-unit treatment, with the effect maintained to the 16 weeks of follow-up. The placebo figure there was high, and the authors still report a significant difference. Several of these trials are linked to the manufacturer. For the odour question, does botulinum toxin help body odour covers what sweat stopping changes.
How long does underarm Botox last?
The median duration of effect was 197 days with 75 units and 205 days with 50 units per underarm in a 52-week trial of 322 people, against 96 days on placebo. That is roughly six and a half to seven months in the middle of the group, and individuals differ. The Chinese trial followed people for 16 weeks and did not report beyond that.
The label says repeat injections should be given when the clinical effect of the previous injection diminishes. In its study, re-treatment was allowed no sooner than 8 weeks after the first, when sweating returned to a severe level. That is a rule for trials, not a schedule for any person. Botox for men covers dose, jaw and sweat questions for men.
What happens at an underarm Botox treatment?
The label describes the procedure, and a good clinic follows something like it. The area to be injected is defined with an iodine-starch test, which stains the part that sweats. Fifty units per underarm is injected into the skin, not the muscle, at 10 to 15 sites about 1 to 2 cm apart, using a fine needle. The label also says units are specific to the product and cannot be converted to another brand.
That last point matters when comparing quotes. A price for a vial or a number of units from two products is not like for like. Ask which product is used and confirm its registration with how to check the product.
When should underarm sweating be checked for another cause?
Sweating that is not primary needs its cause treated first. The Botox label asks that patients be evaluated for secondary causes of hyperhidrosis, giving an overactive thyroid as an example, so that sweating is not treated without diagnosing the underlying disease. Primary sweating is the kind with no other disease behind it, and it is the majority of cases.
Sweating that is new, all over the body, worse at night, or accompanied by fever or weight loss is a reason for a medical check before any injection. Our hyperhidrosis article lists the warning signs, and it is a doctor's task, not a search result's, to sort primary from secondary.
What are the side effects of underarm Botox?
The label lists the commonest reactions in 3% to 10% of adults as injection site pain and bleeding, sweating elsewhere on the body, infection, sore throat, flu-like illness, headache, fever, neck or back pain, itching and anxiety. In the BMJ trial, adverse events were reported by 11% on the toxin and 5% on placebo, a difference that was not significant.
The label also asks for secondary causes of sweating, such as an overactive thyroid, to be considered so that sweating is not treated without diagnosing the cause. It states that the safety of axillary treatment has not been established under age 18.
What other treatments are there for underarm sweating?
Reviews list iontophoresis for palms and soles, oral medicines for resistant or generalised sweating, energy devices such as laser, ultrasound, microwave and microneedle radiofrequency, and surgery. Local surgery for the underarm includes excision, curettage and liposuction. Sympathectomy is the treatment of last resort, because it can cause compensatory sweating elsewhere.
One review cautions that long-term oral anticholinergics such as oxybutynin may carry detrimental effects, including dementia. Each of these has its own evidence and risks, and the choice follows the area, the severity and what has already been tried.
What decides the fee for underarm sweating treatment in Malaysia?
The fee follows the plan: the product and the number of units, the assessment and mapping, who injects, and the follow-up. The US label dose is 50 units per underarm, so the total is double that, and because units differ between products, the same number of units is not the same medicine. How often treatment is repeated also matters, since the effect lasted a median of about 197 to 205 days in one trial.
No figure is given here. Ask the clinic for an itemised quote that names the product, the units, what the assessment includes and what a repeat costs. A quote that states only a price per underarm and no product is hard to compare.
Does The Retreat Clinic in Setia Alam treat underarm sweating?
The Retreat Clinic offers injections for excessive sweating (hyperhidrosis), and the same toxin principle is described on the anti-wrinkle injections page. A doctor first checks that the sweating is primary and not a sign of another condition.
