Quick answer
Freckles can be lightened by lasers, with 69 to 74 per cent clearance after one picosecond session in Chinese studies, but they tend to return with sun, and a new or changing spot needs a doctor first.
- What they are: ephelides are largely genetic and switched on by sunlight; sun spots (lentigines) come from sun exposure and photodamage.
- Laser: picosecond lasers cleared about 69 to 74 per cent of freckles in studies of Asian patients; side effects include dark or pale patches.
- Creams: no freckle-specific cream trial was read; sunscreen matters because sunlight induces ephelides.
- See a doctor first for any new, changing or uneven spot.
What are freckles, and how are they different from sun spots, melasma and Hori's naevus?
Freckles is a lay word for two different things: ephelides and lentigines. A 2014 review says ephelides are largely genetically determined but induced by sunlight, while lentigines are induced by sun exposure and photodamage of the skin. Ephelides are the freckles that run in families, and sun spots are the older, sharper-edged marks that build up with years of sun.
Melasma and Hori's naevus are separate conditions that can look similar. Melasma is a symmetrical patch that darkens with sun, heat and hormones, and Hori's naevus is a scatter of grey-brown spots on both cheeks with the pigment lying deeper. The article on telling melasma, sun spots and Hori's naevus apart sets out the differences, and it matters because a laser that clears one can darken another. The pigmentation page describes how the clinic sorts them.
Do freckles need treatment at all?
Freckles are a normal pigmentation feature, and treating them is a cosmetic choice. The 2014 review describes freckles as important pigmentation characteristics observed in humans, and nothing in the studies read treats ephelides as a disease. What needs attention is a spot that is not an ordinary freckle, which is why the diagnosis comes before any decision about lightening. A person who prefers to leave freckles alone loses nothing medically, and daily sun protection still applies because sunlight keeps inducing them.
Can a laser remove freckles?
Studies on Asian skin report that lasers lighten freckles substantially, and picosecond lasers have been tested most recently. In a split-face trial of 86 Asian participants, one session of a 730 nm picosecond laser gave 68.99 per cent clearance and a 755 nm picosecond laser gave 69.27 per cent, a difference that was not significant. A separate series of 12 Chinese patients reported a mean clearance of 74.46 per cent eight weeks after one session of the 730 nm laser.
These are blinded visual ratings of named devices in small trials with short follow-up. Dr Ong performs PicoSure at the clinic, and the pigmentation page lists freckles among the sun-related marks assessed for it after the diagnosis. The trials do not show how PicoSure performs on freckles, and results from one laser do not carry over to another.
How many sessions do freckles need?
The freckle trials read measured one session, with the result judged at 8 to 12 weeks. How many a given face needs depends on how dark and deep the freckles are, and that is decided at examination. The article on PicoSure sessions, pain and downtime gives the session numbers studies report for other pigment problems, which should not be read across to freckles.
Can laser cause dark or pale patches on freckles?
Yes, and the risk is higher in richly pigmented skin. In the series of 12 Chinese patients treated with a 730 nm picosecond laser, one developed hyperpigmentation (8.33 per cent) and one hypopigmentation (8.33 per cent). A Hong Kong split-face study of 20 Chinese patients compared two alexandrite lasers on freckles and lentigines, found similar improvement with both, and reported post-inflammatory hyperpigmentation after 22 per cent of Q-switched treatment against 6 per cent of long-pulsed treatment, with more pain and redness after the Q-switched laser.
| Study | Laser | Clearance or improvement | Dark or pale patches |
|---|---|---|---|
| Zhang 2023 (86 people) | 730 nm vs 755 nm picosecond | 68.99% vs 69.27% | Not reported in the abstract |
| Huang 2024 (12 people) | 730 nm picosecond | 74.46% | 8.33% dark, 8.33% pale |
| Ho 2011 (20 people) | Q-switched vs long-pulsed alexandrite | Both improved, no significant difference | 22% vs 6% post-inflammatory dark patches |
The article on dark patches after laser covers why this happens, how common it is and what a review found to prevent it, and the article on pigment laser side effects covers the wider risks.
Do creams work on freckles?
No freckle-specific cream trial was read for this article, so no cream can be said to clear freckles. The nearest data are for sun spots. A 2025 review of 41 trials with 3,234 patients found the topical treatment most often reported as effective was a combination of mequinol 2 per cent and tretinoin 0.01 per cent, with efficacy from 52.6 to over 80 per cent for lentigines, particularly on the face, and mostly mild, temporary irritation. Those are lentigo figures and may not apply to ephelides.
In Malaysia tretinoin is a regulated medicine and not a self-service product, as the article on retinoid law sets out. Creams that promise to whiten the skin quickly deserve caution, and a doctor decides what suits the skin.
Does laser for freckles hurt, and what is the recovery?
Studies report discomfort rather than serious pain, with scores that depend on the device. In the 86-person trial, pain scores were 4.69 for the 730 nm laser and 5.65 for the 755 nm laser, and the 12-person series reported 3.08 on its scale. The Hong Kong study found more pain, redness and swelling after the Q-switched laser than after the long-pulsed one. The skin can look red or darker for a period afterwards, and the dark phase is the point at which sun protection matters most.
How reliable are the freckle laser studies?
They are small and short, so the figures show what is possible and not what any one person will get. The trials read had between 12 and 86 people, most measured one session and followed people for 8 to 12 weeks, the clearance was rated visually, and each tested named devices. The lentigo review, which is the larger body of evidence at 41 trials, still calls for large randomised trials. None of them gives a recurrence rate, so the figure to plan around is the one the mechanism suggests: freckles can return when the skin is exposed to sun again.
Why do freckles come back?
Freckles come back because sunlight induces ephelides, and treatment does not change the skin's tendency to make them. A 2014 review calls them largely genetically determined but induced by sunlight. No study read follows treated freckles long enough to give a recurrence rate, so none is given here, but the mechanism points to daily sun protection as the main lever. The article on sunscreen in a tropical climate explains what the SPF number does not tell you, and sunscreen was the one step that consistently prevented post-laser dark patches in the review described in the dark patches article.
When does a spot need a doctor before any laser?
A spot needs a doctor first if it is new, changing, uneven in colour or border, raised, bleeding, or alone on one side of the face. A 2022 systematic review states that telling early melanoma from other flat pigmented lesions on the head and neck is challenging, both clinically and with a dermatoscope. A spot should be diagnosed before it is treated, and the clinic's pigmentation page says an atypical or changing spot is referred for a specialist opinion before any aesthetic treatment is considered.
Which other treatments appear in the freckle and sun spot studies, and which does the clinic offer?
The lentigo review also lists intense pulsed light, pulsed dye laser, fractional CO2 laser, cryotherapy and trichloroacetic acid peels, with success rates from 8 per cent to 90 per cent depending on the method. The clinic does not offer intense pulsed light, vascular lasers or fractional CO2. Dr Ong performs PicoSure, and the first step is always the diagnosis, which may end with sunscreen and a cream and no laser.
