Anti-ageing treatment in Setia Alam: what a doctor decides before any machine

Dr Ong Jin Khang · MMC 45245 · LCP holder · The Retreat Clinic, Setia Alam

Before any machine or injection, a doctor decides which of five things is ageing the face: lost volume, lax skin, skin quality, pigment or expression lines. Each has a different fix, and no single treatment covers all five. The evidence for each option is real but specific, such as an 86 percent brow-lift rating for ultrasound at 90 days in one small study and 24 percent less skin ageing with daily sunscreen over 4.5 years.

Ageing is five findings.

What does a doctor decide before any anti-ageing treatment?

Which of five things is ageing the face, because each one has a different fix. They are lost volume, lax skin, changes in skin quality, pigment and expression lines. A machine chosen before that question is answered is a guess, and the essay there is no anti-ageing treatment, just specific findings makes the same point at greater length.

People searching for anti-ageing treatment in Setia Alam, an anti-ageing clinic in Shah Alam or rawatan anti penuaan are usually asking one question in three languages: what should be done, and by whom. A first visit at a doctor-led clinic is where the assessment comes before the treatment.

What are the five things that age a face, and how is each assessed?

A classic review describes facial ageing as a synergy of changes in the skin and loss of facial volume, driven by gravity, bone resorption, reduced elasticity and shifting of fat. It assesses the face in thirds: forehead and brow, midface, and chin, jawline and neck. That gives a frame for the five findings.

Volume is lost fullness, assessed by looking at the cheeks, temples and under-eyes from the side and in movement. Laxity is skin and tissue that no longer holds its position, assessed at the brow, jawline and neck, and it is the finding that devices target. Skin quality is texture, hydration and fine surface lines, assessed up close. Pigment is sun spots, melasma and uneven tone, assessed by type and depth. Expression lines are creases made by muscles, assessed at rest and in movement. Most faces carry more than one finding, in different proportions.

Which option matches which finding?

Anti-wrinkle injections match expression lines. In two randomised placebo-controlled phase III trials with 362 and 368 participants, botulinum toxin was significantly more effective than placebo for forehead, frown and crow's feet lines at Day 30. The page on anti-wrinkle injections covers the clinic's approach, and which treatment fits which line goes line by line.

Dermal fillers match lost volume. In a randomised, evaluator-blinded study of 148 Chinese subjects, 76 percent of those given a hyaluronic acid filler were responders at 6 months against 8 percent of untreated controls, and 51 percent were still responders 12 months after the last injection.

Skinboosters match skin quality. A systematic review of 13 studies found improvements in hydration, firmness, texture, brightness and elasticity after injectable hyaluronic acid, and the authors say larger randomised trials are still needed.

Biostimulators match gradual volume loss. In a randomised, assessor-blinded trial of 331 people, poly-L-lactic acid reached 90.57 percent responders on a midfacial volume scale against 51.01 percent for hyaluronic acid, and 84.91 percent against 46.98 percent at 12 months. Injection-site reactions were slightly more frequent and cleared within 1 to 3 days. The page on biostimulators explains how the class works.

Ultherapy matches early laxity at the brow, jawline and neck. In an uncontrolled, rater-blinded cohort of 35 people, three masked clinicians judged 86 percent to show a clinically significant brow lift at 90 days, and mean brow height rose by 1.7 mm. That is a modest, measured lift, not a surgical one.

XERF is radiofrequency, and radiofrequency has the broadest evidence. A systematic review of 121 articles found lifting, better elasticity and collagen, and wrinkle reduction, with higher satisfaction when the aim was modest. No XERF-specific trial was read for this page, so the claim is made for the class. Eyelid skin is not treated with XERF or Ultherapy at present.

PicoSure matches pigment. A meta-analysis of 20 randomised trials and one retrospective study (971 patients) found picosecond and nanosecond lasers cleared endogenous pigment comparably, with fewer post-inflammatory pigment changes after picosecond treatment. Those changes were more obvious in Asian patients, which is why the pigment type matters before a laser is chosen.

Which anti-ageing steps are not a machine at all?

Sunscreen and retinoids. In a randomised trial of 903 adults younger than 55 in Queensland, Australia, followed for 4.5 years, skin ageing was 24 percent less with daily broad-spectrum sunscreen than with discretionary use (relative odds 0.76, 95 percent CI 0.59 to 0.98). Some outcome data were missing, so the figure is an estimate. The Malaysian side of sunscreen is covered in the related page below.

Retinoids have the longest record. A review of the literature found tretinoin the most investigated retinoid for photoageing, with irritation the main limit and retinaldehyde and retinol gentler. A doctor can match the retinoid to the skin.

What does not work?

A treatment chosen for the name rather than the finding does not work. Volume loss will not be fixed by a pigment laser, and expression lines will not be fixed by a booster. In the sunscreen trial, beta-carotene supplements had no overall effect on skin ageing, which is a useful reminder that a supplement is not a treatment because it sounds protective.

Two other categories have their own pages on this site: at-home HIFU, radiofrequency and LED devices (linked below) and collagen drinks and supplements. Neither replaces a diagnosis.

How do you check an anti-ageing provider?

Ask who the doctor is, and check the registration. The guide on how to check an aesthetic doctor in Malaysia shows the register checks. Ask who performs each procedure: at The Retreat Clinic, Dr Ong performs injectables, Ultherapy, XERF and PicoSure. Ask what the examination found before any option was offered, and whether the answer to your concern is sometimes no treatment.

The Retreat Clinic is on Jalan Setia Dagang in Setia Alam, five minutes from Setia City Mall, and outside clinic hours it answers on WhatsApp only.

Common questions

What anti-ageing treatment is there in Setia Alam?

A doctor-led clinic in Setia Alam can offer anti-wrinkle injections, dermal fillers, skinboosters, biostimulators, Ultherapy, XERF and PicoSure. Which of them suits a face depends on which of the five ageing findings is present: volume, laxity, skin quality, pigment or expression lines.

Which anti-ageing treatment works for the face?

No single treatment covers all five findings. Ageing reflects skin changes and loss of volume together, with gravity, bone loss and reduced elasticity, so the answer depends on what the examination finds.

Does daily sunscreen slow skin ageing?

In a randomised trial of 903 adults followed for 4.5 years, skin ageing was 24 percent less in the daily sunscreen group than in the discretionary group. Beta-carotene supplements had no overall effect.

Are Botox and filler the same as anti-ageing treatment?

They treat different findings. Botulinum toxin (Botox) softens expression lines, and in two phase III trials it beat placebo on forehead, frown and crow's feet lines at Day 30. Hyaluronic acid filler replaces lost volume, and in a randomised study 76 percent of treated people were responders at 6 months against 8 percent of controls.

How do I check an anti-ageing clinic in Shah Alam or Setia Alam?

Confirm the doctor is a registered medical practitioner, ask who performs each procedure, and ask what the examination found before anything was suggested. The guide on how to check an aesthetic doctor in Malaysia sets out the register checks.
