What do most people notice after Ultherapy?
The usual list is short. A review of the safety literature names tenderness, redness and slight swelling as the typical events, and a systematic review of 19 articles covering 506 patients found transient swelling, redness and pain most often. A rater-blinded cohort of 35 people reported that most responded with mild, passing redness and swelling.
Less often there are bruises, welts (raised lines that settled without treatment in 3 of 93 people in one study) or a patch of tingling or numbness near a nerve. Because the surface of the skin is not broken, there is no wound to dress. What Ultherapy does happens below the surface, which is why the treatment page describes it as a heating effect at depth and not an injury you can see.
How long do redness, swelling and tenderness last?
Studies give days, not weeks. In a 2026 pilot of 21 patients, swelling lasted 3.95 days and redness 1.05 days on average. That pilot used a different micro-focused ultrasound system and had no comparison group, so it describes a range more than it promises one. In a 22-patient physiology study, swelling resolved without sequelae and skin temperature showed no significant rise by day 3.
The 2014 FDA clearance summary for the Ulthera system (K134032) reports no serious adverse events and says that all but two of the adverse events were mild, but it gives no day counts, so no label timeline is quoted here. Tender spots at depth, tingling and the odd welt tend to take longer than redness, and the side effects article covers how long those can take.
Is numbness or tingling normal?
A patch of altered feeling is a recognised effect. The safety review lists nerve-related effects, paraesthesia and weakness, as rare, and the more recent systematic review found focal numbness and nerve damage among the more frequently reported entries in the US device-event database. Those database entries are reports, not rates, so they cannot say how common it is.
The practical line is the one the reviews draw. Mild tingling that fades is within what is described. Numbness that spreads, stays, or comes with a droop in the smile or brow is not something to wait out, and Ultherapy and HIFU side effects sets out that list in full.
What helps, and what is only convention?
The honest answer is that no trial read for this article tested cold packs, creams, sleeping position or any other routine after Ultherapy. What has been measured is that the skin barrier is left alone: in the 22-patient study, water loss, hydration and redness were unchanged from baseline over the follow-up. That is why there is no wound-care routine to follow.
Everything else is convention. Gentle skincare while the area is tender, sun protection as a sensible habit on any treated skin, and avoiding extra heat on a tender jawline are common advice and cost nothing, but they rest on general practice, not on a result from these studies. At The Retreat Clinic the aftercare after Ultherapy is the usual routine described here, with nothing special added.
When do results show?
Not in the first days, which is part of why this is a quiet treatment. In the physiology study, skin elasticity measures dipped at week 4 and were higher than baseline by weeks 12 and 24, which fits a remodelling process measured in months. How long Ultherapy lasts, and when repeat treatment helps covers the timeline and when a second session is worth discussing, so it is not repeated here.
A different sort of difference is worth knowing about. A 2025 survey of 231 people treated with two generations of the device scored pain, redness and swelling lower with the newer system, so what you notice afterwards also depends on the device generation. That survey was one clinic and unblinded.
When should I call?
Contact the clinic promptly for blistering, broken or crusting skin, pain that is severe or getting worse, a droop or unevenness of the smile, brow or lip, numbness that spreads or does not settle, or hollowing in an area that was fuller. A case series of five patients described blistering, erosion or ulceration, swelling with later loss of tissue and skin death after single sessions. The authors call this rare and possibly underreported, and say early assessment matters.
Ultherapy is offered and performed by Dr Ong at The Retreat Clinic in Setia Alam. After hours, WhatsApp is the way to reach the clinic with a worry. Knowing what is expected before the day, and what is not, is part of consenting to any treatment.
